Company History

EnVogue Med & Wellness (EVMW) was founded in November 2022 by a prominent local family physician Dr. Sandy Valino Stock with the premise that patients are customers and personalized healthcare can be delivered in a more efficient, cost-effective manner. The low cost membership model supports timely communications and quick medical questions to your physician that otherwise would not be covered on most health insurance plans. EnVogue Med & Wellness seeks to continually streamline its processes so the patient journey is excellent throughout.

Did you know, it takes on average, 20 days to secure an appointment with the average U.S. primary care physician? Once seen, appointments are usually rushed. With EnVogue Med & Wellness, you can secure appointments usually same or next business day. Also, your appointments are not rushed.

In 2024, En Vogue Med & Wellness is now accepting telemedicine membership in the states of Illinois, Wisconsin, California, and Florida.

Appointments

We know you have many choices when choosing a primary care physician in Schaumburg, IL so we have made requesting an appointment a simple process via our website or via your Elation Passport app/portal. If you cannot keep a scheduled appointment, please reschedule ASAP via the Elation Passport app or text us at (847) 916-0673.

Billing and Insurance

EnVogue Med & Wellness is a concierge-style medicine practice. This means there is a reasonable monthly membership fee in addition to the cost of the medical services we provide. This model ensures your provider can deliver more convenient and attentive care for you and your family.

We now accept Medicare Part B, BCBS PPO/POS, Aetna POS/PPO, United Health Care PPO, and Cigna PPO/POS. As this time we DO NOT accept HMOs, Medicaid, or Medicare Advantage.

We also provide transparent pricing for our self-pay members. Of note, we highly recommend our members to have health insurance to cover hospital services, specialists, medications, tests, and imaging not offered by our practice. We are not an insurance plan or a replacement for health insurance.

Payment Options

We use a secure electronic payment system for debit/credit cards including FSA/HSA cards(if applicable), and e-checks. Please contact us if you have any questions or need to make other arrangements. For security, we do NOT have cash on hand.
